Projects for Assistance
in Transition from
Homelessness (PATH)

The Projects for Assistance in Transition from Homelessness (PATH) program provides free, intensive case management services to adults diagnosed with serious mental illness who are homeless or at imminent risk of homelessness and are not receiving other case management services. PATH connects individuals with essential resources and support systems to address immediate needs and promote long-term stability.

Through strong community partnerships and a person-centered approach, PATH helps individuals navigate barriers such as lack of housing, untreated mental and physical health conditions, and limited access to benefits. Each connection made through PATH is a step toward dignity, wellness, and recovery.

Enhancing Quality of Life

Reducing homelessness, expanding access to care, and strengthening the well-being of vulnerable adults begins with addressing the social, emotional, and environmental factors that influence recovery.

Services

PATH provides outreach-based, wraparound support that bridges critical gaps for individuals experiencing both homelessness and mental illness.

Linkages to Care

Connections are made to vital services such as primary care, mental health treatment, substance use recovery, and social supports.

Housing Assistance

Individuals receive help locating housing opportunities and completing necessary applications, along with advocacy and follow-up support.

Life Skills Development

Fosters relationship-building with family and natural supports while connecting individuals to employment, education, and financial assistance.

Outreach & Engagement

Case managers meet individuals in homeless shelters, soup kitchens, hospitals, and on the street, building rapport and linking them to services.

Program Eligibility Requirements

Adults ages 18 and older who reside in Monmouth County may be eligible for services if they are currently experiencing homelessness (or are at imminent risk) and have been diagnosed with a serious mental health condition. Individuals must not be actively engaged in other case management services at the time of referral.

Many who qualify face co-occurring medical conditions, substance use, and long-standing barriers to care, including lack of financial benefits and limited support systems.
