
Housing Supports
Program
The Housing Supports Program helps individuals and families enrolled in New Jersey Managed Care Organizations (MCOs) through NJ FamilyCare secure and maintain safe, stable housing. Using a person-centered and trauma-informed approach, HSP provides assistance with housing searches, applications, and tenancy-sustaining supports such as budgeting, daily living skills, and community connections.

Building on the Mental Health Association of Monmouth County’s experience in case management and homeless services, HSP strengthens housing stability and promotes long-term wellness for members across Monmouth County.

Pre-Tenancy Services
Assistance with housing searches, application support, documentation, and financial planning.
Tenancy Sustainment
Case management focused on daily living skills, budgeting, rental arrears, and connection to resources.
Collaborative Care
Coordination with MCOs, landlords, and local service providers to promote housing retention and stability.

Program Eligibility Requirements
The Housing Supports Program serves individuals and families enrolled in a New Jersey Managed Care Organization (MCO) through NJ FamilyCare who are experiencing homelessness or at risk of losing their housing.
