Free, individualized support for people and families in Monmouth County who are finding or keeping a stable home.

Program Eligibility Requirements

The Housing Supports Program serves individuals and families enrolled in a New Jersey Managed Care Organization (MCO) through NJ FamilyCare who are experiencing homelessness or at risk of losing their housing.

Supporting Housing Stability

We help individuals and families secure and maintain stable housing through coordinated support, resource connection, and skill development, empowering them to overcome barriers and build lasting independence.

Services

The Housing Supports Program provides flexible, individualized services designed to meet members where they are in their housing journey.

Pre-Tenancy Services

Assistance with housing searches, application support, documentation, and financial planning.

Tenancy Sustainment

Case management focused on daily living skills, budgeting, rental arrears, and connection to resources.

Collaborative Care

Coordination with MCOs, landlords, and local service providers to promote housing retention and stability.

Kayaking with Family Dog
