The Housing Navigation Program (HNP) helps individuals and families experiencing homelessness secure permanent housing and transition out of the homelessness system. As part of the Monmouth County Continuum of Care (CoC), HNP provides essential housing assistance and serves as a critical point of exit for individuals facing homelessness.

 

The program is committed to helping clients navigate the complexities of the housing system, ensuring they have the support and resources needed to maintain long-term housing stability.

We aim to reduce homelessness by prioritizing and assisting individuals and families in obtaining permanent housing, offering a pathway out of homelessness, and creating lasting stability through coordinated community efforts.

Coordinated Entry

Receives referrals from MCDSS’s Central Intake Unit, homeless shelters, and the PATH program, ensuring individuals are prioritized for housing assistance and connected to the right resources for their needs.

Housing Assistance

Helps individuals navigate the housing system, identifying suitable permanent housing opportunities and providing case management to ensure successful transitions into stable living environments.

Outreach & Engagement

Conducts outreach efforts, especially to vulnerable populations, including those in encampments or at risk of homelessness, connecting them to housing services and other essential support systems.

Community Collaboration

Partners with municipal authorities, community providers, and local agencies to address housing challenges, ensure coordinated care, and advocate for affordable housing options.

Program Eligibility Requirements

The Housing Navigation Program (HNP) provides services to adults and families who are experiencing homelessness in Monmouth County. Referrals are made through the Monmouth County Division of Social Services' Central Intake Unit (MCDSS), homeless shelters, or the PATH program. HNP is part of the Monmouth County Continuum of Care (CoC) and is funded by the U.S. Department of Housing and Urban Development (HUD), supporting the region’s efforts to reduce homelessness and improve housing access.

HNP works in collaboration with community partners, shelters, and the PATH program to identify and prioritize individuals for permanent housing. By addressing immediate housing needs and providing ongoing support, the program helps clients build the personal growth and financial stability essential for long-term recovery and well-being.
